Community Care Hub Indiana (CCHI) centralizes administrative functions and operational infrastructure, including but not limited to contracting with healthcare organizations, payment operations, management of referrals, service delivery fidelity and compliance, technology, information security, data collection, reporting, and business development and innovation. Through an integrated care model and data-driven approach, CCHI will elevate the State’s CBO network for better patient health outcomes at lower costs and quicker response times

 

Statewide Network Standardization: CCHI utilizes standardized evidence-informed or evidence-based interventions and care coordination models that align with National Committee of Quality Assurance (NCQA) principles for person-centered care. The CCHI will ensure network standardization for all processes with CBOs through its processes. 
 

Technology system alignment: CCHI’s technology platform is HITRUST certified and provides for the secure electronic transmission of data between networks and provides analysis of outcomes to ensure quality outcomes.

Better ROI for Indiana: The State of Indiana managed care market is a 14-billion-dollar industry, and costs are expected to increase as Indiana’s population ages. The Community Care Hub of Indiana contracts with healthcare systems, managed care organizations, the State of Indiana, and private businesses to support their patients, members, and employees to support social determinants of health and expand better health outcomes at lower costs for all. This approach will allow Hoosiers to age in place longer, lower overall healthcare costs, provide an attraction and retention model for retirees and create a multi-billion dollar economic boom for aging-in-place initiatives with Indiana businesses.
